BIOFIRE FILMARRAY SYSTEM is a FDA-approved multiplex PCR system having 4 different panels: CSF Panel, Blood Culture Panel, Respiratory Panel and G.I. Panel. BIOFIRE GI Panel is for detection of causative organism of diarrhoea due to bacteria, viruses or parasites. It includes 12 bacteria (Campylobacter, Clostridium difficile, Plesiomonas shigelloids, Salmonella, Yersinia enterocolitica, Vibrio cholerae and other non-cholerae Vibrio species, EAEC, ETEC, EPEC, EIEC, Escherichia coli O157, STEC), 4 parasites (Cryptosporidium, Cyclospora cyatenensis, Entamoeba histolytica, Giardia lamblia) & 5 viruses (Adenovirus, Astrovirus, Noravirus, Rotavirus, Sapovirus). Whereas bacterial diarrhoea needs specific treatment, viral or parasitic GI Infections are completely different. Parasitic diarrhoea occurs in immunocompromised patients as opportunistic infections and requires different antibiotic and viral diarrhoea is highly contagious, leads to hospital-acquired infections. Rice watery diarrhoea due Vibrio cholerae infection needs urgent treatment and isolation to reduce morbidity and mortality of the patients especially in paediatric patients. It can also diagnose enteric fever/typhoid fever. BIOFIRE GI Panel gives the diagnosis of the organism only in 2 hours. Stool or Rectal swab is to be collected in a sterile, leak-proof, screw-capped universal container and to be sent to the laboratory at the earliest. It is a gold standard technique for the detection of the pathogen with the sensitivity of 96% and specificity of 99%.
